This form will be given to an applicant if a potential employer, based on information in a pre-employment credit report or information in a pre-employment screening report, takes an adverse action, not hiring the employee candidate.

Notice of Adverse Action

Neither the credit reporting company nor National Intelligence Service, Inc. made the decision resulting in the adverse action and cannot explain why the action was taken.

If information on your pre-employment credit report resulted in the adverse action and you dispute the accuracy or completeness of the information, you can receive a copy of the report from the company representative, if desired. You may also obtain a free copy of the credit report from the credit reporting company if requested within sixty (60) days of the adverse action. Information is included on the credit report advising you of the procedure to contact the credit reporting company directly to dispute the accuracy or completeness of any information on the credit report.

If information in the pre-employment report for your application was the basis for adverse action and you dispute the accuracy of the information, please call the number below within sixty (60) days of being advised of the adverse action:

Public record information that is disputed by you will be re-confirmed with the appropriate sources(s). If the information is still disputed by you, you will be referred to the appropriate public record source (court records) to discuss correction, if appropriate. Please be advised that fingerprints taken at a police station may be required if a criminal record is disputed.
